Pre-Admission Registration

Thank you for choosing Jackson Hospital for your healthcare. Even before you arrive, our staff is preparing for your visit and we hope that your stay will be as comfortable and pleasant as possible!

Pre-admission registration allows you to fill out basic information ahead of time and saves you time the day of your procedure. During pre-admission registration, take note of the following items:

• Co-payments for your procedure. Check with your insurance provider to fully understand financial responsibility.
• Bring important documentation such as your insurance card.
• Confirm your appointment time. Familiarize yourself with parking and where to go once you arrive.
• Ask questions. Call pre-admission registration to ask any questions you may have prior to your procedure.
