Health Risk Assessments

A Health Risk Assessment (HRA) is an online questionnaire tool used to identify risk factors for certain illnesses or disease based on your unique health history. An HRA can be used to take proactive steps in bettering your health, such as making lifestyle changes or seeing your healthcare provider for next steps in your care.

If the Health Risk Assessment identified any potential risks, it's important to contact your healthcare provider to discuss your risks in greater detail. Visit our Provider Directory and sort by specialty to find a provider that fits your needs.
