A Health Risk Assessment (HRA) is an online questionnaire tool used to identify risk factors for certain illnesses or disease based on your unique health history. An HRA can be used to take proactive steps in bettering your health, such as making lifestyle changes or seeing your healthcare provider for next steps in your care.

How can I use this report to improve my health?

The report provides you with a summary of your results, isolates your key risk factors, and helps you understand what you can do to improve your health. The report also contains two other sections:

  • Information About Risk Factors: This section provides personalized information about each of the risk factors based on your answers to the questions. You will also find information outlining what you can do to reduce your risk.
  • Improving Your Health: This section gives your information on ways that Jackson Hospital and our affiliated providers can help you begin to reduce and manage your risk factors.

What information do I need to complete a Health Risk Assessment?

You will need to provide basic background information that may include family medical history, personal medical history, current symptoms you may have, or pre-existing conditions. Don't worry if you don't know the answers; you can skip them or provide your best estimate.

How will I receive the results for my Health Risk Assessment?

The results of your completed HRA will be sent directly to your email address. You will need to provide some basic information at the start of your assessment, but all of this information is kept private.
